Mark Lamos, M.D.Dr. Lamos is the President and Medical Director at Greater Baltimore Medical Associates. His extensive experience includes daily patient care as well as quality assurance, information systems and insurance contracting which made him the natural selection to serve as the Innovative Lead for Transformation of Primary Care into a single operative unit with responsibilities for the Outpatient Service Line of GBMC.Additionally, Dr. Lamos has recognized the need to transform his group into a Patient Centered Medical Home to better care for older, medically fragile patients. The patients require coordination between primary care, specialists and other healthcare services to better manage what is usually several chronic, pre-existing conditions.Dr. Lamos is often described as s physician that offers consistent leadership and a strong moral compass. He has held several teaching positions and has served as Clinical Teaching Faculty at GBMC since 1990.Irina Koyfman, NPIrina is a Nurse Practitioner who serves as the Director of Transitional Care at Maxim Healthcare. Her 15 years of clinical experience combined with her strong background in executive leadership and management make her the natural choice to lead University of Maryland St. Joseph Medical Center’s newest initiative called Maxim Transition Assist.Her expertise in coordinating clinical, psychosocial and post-acute care activities to provide at-risk patients with the highest levels of care and support they need during the critical 30 day post-hospital discharge period when at-risk patients are most susceptible to readmission. Managing a program like this task requires the highest levels of collaboration, team building, communication, teaching and mentoring skills and Irina has mastered them all.Sean HullSean began his career over 20 years ago and found great success as wealth strategist providing holistic, personal advisory services to a variety of clients including professional athletes, entertainers and entrepreneurs. His professional demeanor and problem solving abilities are directly attributed to his experience in the wealth accumulation, financial planning, insurance, trust and estate planning disciplines within the financial services industry.Additionally, Sean is active on the NCAA College basketball courts as a men’s basketball official in the Atlantic Coast Conference and many other conferences.In 1996 his mother, Ida, passed away suddenly after a long and unknown battle with Sarcoidosis. The way Ida lived her life was a great lesson to Sean. She taught him that we must accept the things we cannot change and fight hard to change the things we can. From this lesson grew the Life and Breath Foundation. Sean was inspired to help others in memory of a woman who had dedicated her life to helping and supporting others – especially her family.Edward S. Chen, M.D.Dr. Chen is a pulmonologist and critical care physician at Johns Hopkins Hospital and Johns Hopkins Bayview Medical Center. He specializes in pulmonary diseases including sarcoidosis and interstitial lung disease. In 2010 Dr. Chen was the lead investigator in a study of the link between sarcoidosis and the overproduction of the suspected protein trigger, called serum amyloid A.Additionally, Dr. Chen serves as Assistant Professor of Medicine at the Johns Hopkins School of Medicine.
